To register your account, please follow the steps below:
- Enter your Account Number or DoctorID. Your Account Number and DoctorID are printed on your Invoices and statements.
- Enter your Phone Number
- Enter the last Payment Amount (if prompted)
From the right side of the form:
- Choose User Name and Password. We recommend both of them to be at least 6 character long.
- Enter your email address
All these fields are required.
